Progress Notes - 12/26/02            Temp  103.8

Finale is a 6yr old doe presented today with a 5 day history of being unable to walk normally.  The owner noticed her on Sunday 12/22/02 leaning on a hay feeder.  She has slowly gotten worse.  The owners have found her leaning to the left side and walking toward the left, not putting as much weight on the right side.  She is eating hay and rain well, but not drinking much.  She is currently 2 –2 ½ months pregnant.  She has a skin problem, which is resolving.  It started approx. 2 mo. Ago with what was thought to be a scratch on her neck, which was very pruritic and it spread to her chest.  Owner applied Blue Kote after all other efforts failed to clear it up and it seemed to help or the problem was resolving already.

(Pictured to the left is finale and a new lesion on her right side, note the straight line appearance) She is in the back seat of the car on her way to Ohio State, she could not walk at this point.

Observations:  T= 103.8, P= 180, R=36 She was down on admission and unable to rise, although she would move her legs (front primarily to try to get up and seemed a bit ataxic.  She had an intention tremor (slight) in her head and twitching of her neck muscles.  PLR’s were sluggish.  Panniculus reflex was difficult to assess, as it was present in some areas and absent in others.  But it generally seemed less caudal to the Thoracolumbar yxn.  She had tail and anal tone.  Alopecia was present on left neck extending the full length of the neck and she had a strip of alopecia on her right flank.  She was pruritic.

Assessment: DDX: Brain abscess, Spinal trauma, Bacterial Meningitis, CAE, Polioencephaolmalesia, Listeria, lead toxicity, scrapie, Meningeal worm.

Treatment:     Nuflor 2700mg SQ x 1 dose (to last 4 days)

                        Dexamethasone 10mg SQ SID

                        Thiamine 3ml SQ SID x 3 days

                        Panacur 250lb dose SID x 4 days

Additional:     Thoracolumbar X-rays – no lesions

                        CBC: dehydration, anemia, neutrophilia

                        CSF cyto, WBC=1125/ul, RBC = 212, 400/ul – unsure whether this is due to blood                             contamination or chronic inflammation (i.e.. Meningitis) 

Progress Notes – 12/27/02  Temp 101.3

Finale is a 6yr female goat presented yesterday for neuro problems (being down, ataxic, unable to get up) for 5 days duration.  Today, after being helped to stand, she stood for 30 seconds then fell to her left side.  She is ataxic in all 4 limbs and weak in her rear legs.

T= 101.3, P= 132, R= 40, heart and lungs auscultation normal.  Normal ruminations, eating very well.  Still some tremors of the limbs and head.  CSF analysis – blood contamination, possible meningitis

Assessment: Seems to have slightly improved today.  Still cannot get up on her own Most likely DDX, CAE, meningitis, CNS mass (abscess or neoplasia), Meningeal worm. 

Treatment:    3cc Thiamine, SQ

                        5cc Dexamethasone SQ

                        250lb Panacur Paste PO

                        2 ml Ivomec SQ 

CSF profile:  CK – 1181 – consistent with traveling Meningeal worm.

                        Cell counts: 33% Eosinophils – possibly parasites (Meningeal worm) 

Progress Notes:  12/28/02,  Temp  101.3

Finale seems to e a little stronger but cannot stand in the rear with out assistance or stay standing.  She’s eating well. 

T = 101.3, P = 120, R = 36,  heart and lungs normal with normal ruminations. 

Waiting to see if treatments improve neuro state. 

Treatment:     5cc Dexamethasone SQ

                        3cc Thiamine  SQ

                        250 lb dose panacur paste PO

 Progress Notes :  12/29/02,  Temp 101.0

This morning, after being stood with assistance, Finale stood for 1 minute before laying down on her left side. 

T = 101.0, P = 120, R = 36, heart and lungs normal with normal ruminations

Neuro  status slightly improved 

Treatment:            5cc Dexamethasone SQ

                        3cc Thiamine  SQ

                        250 lb dose panacur paste PO (last dose) 

Progress Notes :  12/30/02,  Temp 101.5

Finale is a 6yr old female dairy goat with a 9 day history of being unable to walk or get up on her own. 

P = 132, R = 30, she still has the intention tremor of her head, and is very pruritic (she has a history of skin disease that is resolving but unknown cause)  She does seem stronger today and can ambulate around the stall with only support of her tail and hind limbs. (holding her up with her tail and she walks close to the wall for support) 

Assessment:  Her neurological status appears to be slowly improving – diagnosis = Meningeal worm due to high eosinophil count of CSF.  Need to access skin condition.

 Treatment:    5cc Dexamethasone SQ

                        3cc Thiamine  SQ

                        Get her up a couple times a day either in sling or just standing with support.  Need to have Dr. Hull reassess her today and determine need to continue medications. 

Improvement was noted by Dr. Hull.  Finale was able to stand on her own while leaning against the wall for 1 minute. 

Progress Notes :  12/31/02

Finale is a 6yr old female dairy goat who presented at OSU 5 days ago for posterior ataxia and weakness.  Primary differential is Meningeal worms based on a high percentage of Eosinophils in the CSF. 

P = 128, R = 40, Intention tremors of the are present.  She attempted to rise on her own but was only able to push herself into a dog sitting position, then immediately slumped back to the floor.  Appetite good, Attitude – QAR.  She was able to stand with tail support and she stumbled around the stall.  Her strength and coordination are slowly improving. 

Finale continues to improve neurologically.  Primary ddx is still Meningeal worms.  Panacur treatment is completed, continue treatment for other diagnosis.  Skin condition has not been evaluated 

Treatment:     5cc Dexamethasone SQ  (last dose)

                        3cc Thiamine  SQ (last dose)

                        Walked with support three times today
